二、工作内容

1.总体布局、生产工艺和设备布局、建筑卫生学要求、职业病危害因素和危害程度及对劳动者健康的影响、职业病危害防护设施及效果、辅助用室基本卫生要求、应急救援、个人使用的职业病防护用品、职业卫生管理等。

2. 现场职业卫生学调查，了解建设项目生产工艺过程、劳动过程、生产环境，确定所存在的职业病危害因素，检查职业病危害防护设施的落实及职业卫生管理的实施情况。

依据国家有关职业卫生的法律、法规和技术规范、标准，以及操作规程、职业病危害事故案例等，通过对拟评价项目的详细分析和研究，列出检查单元、检查部位、检查项目、检查内容、检查要求等，编制成表，逐项检查符合情况，确定拟评价项目存在的问题、缺陷和潜在危害,必要时可进行量化分析。

3.依据国家相关技术规范和标准的要求，通过现场检测和实验室分析，对拟评价项目作业场所职业病危害因素的浓度或强度以及职业病危害防护设施的防护效果进行评定。

三、进度要求

乙方应于合同签订后，在____年____月_____日前完成______________项目职业病危害现状评价报告书的编制工作。

四、文件份数

乙方应向甲方提供职业病危害现状评价报告书8份。

五、双方义务与责任

1.甲方的义务与责任：

1.1 甲方应提供与评价项目相关的下列资料，并对所提供资料的可靠性和准确性负责：

1.1.1 建设项目总平面布置图、车间平面布置图。

1.1.2建设项目工艺流程简介和工艺流程图。

1.1.3原料辅料使用情况(组成、用量等)，生产工艺流程中存在化学物料名称和接触情况。

1.1.4建设项目设备情况。

1.1.5 建设项目职业卫生防护设施：如通风除尘排毒系统设计性能、车间通风换气(全面通风换气量、空气调节及洁净区参数)、事故应急求援设施(紧急救援站的面积、冲洗装置等)、污水处理系统等。

1.1.6 职业卫生管理措施：生产制度、劳动定员、安全操作规程、应急救援预案、作业工人的个人防护、年度职业病防治计划、培训制度、健康监护制度、有害因素监测制度等。

1.1.7 卫生辅助设施：工作场所办公室、更衣室、盥洗室(盥洗水龙头个数)、浴室(淋浴器的个数)、宿舍、食堂的设置情况。

1.1.8其他资料：该项目职业卫生监测档案，职工健康监护档案等。

1.2 根据本合同规定向乙方支付评价费用。

1.3 为乙方到现场工作的人员提供相应的必要的个人防护装备。

2.乙方的义务与责任：

2.1 乙方按第三条规定按时向甲方提交评价报告。

2.2 乙方对评价报告文件质量负责。

2.3 乙方对甲方提供的技术资料负责保密，未经甲方许可不得向第三方泄漏。

六、评价费用和支付方式

根据《 省卫生防疫收费标准》中“劳动卫生学评价”有关规定，经甲乙双方友好协商确定合同总价款为人民币：_______万元整(_______元)，合同生效7日内，甲方向乙方支付合同款的50%，即人民币：______元整 (_____元)，作为前期工作费用。在乙方交付职业病危害现状评价报告，并通过甲方验收后，甲方一次付清合同余款，即人民币：____元整 (______元)。
