二、参营内容及安排

三、甲方责任

甲方替乙方购买人身意外保险。

四、乙方责任

乙方入营后，应以甲方辅导员老师为核心，一切活动必须服从甲方老师安排。

乙方须如实、完整地填写报名信息，向甲方履行告知义务，并保证学员身体健康，无隐患疾病(如心脏病、高血压、传染病等)，同时还应承诺：保证乙方学员的身体状况能够适应甲方主办的夏令营活动，如在活动期间乙方因自身疾病而引起的医疗、抢救和住院等费用，全部由乙方自行承担。

乙方必须遵守团队行程活动计划，全部行程必须随营集体活动，未经允许，不得擅自离营单独活动。如出现违反上述规则或甲方规定现象，责任自负。甲方将不承担或退还任何费用。

乙方如违反国家相关法律法规以及社会治安管理条例或不遵守夏令营纪律和相关规定，乙方应自行承担因此而产生的一切责任，同时甲方有权对其进行劝导，情节严重的可令其提前离营并不退还任何费用，所产生的一切后果和费用乙方自负。

在夏令营活动期间，如果因乙方学员个人原因而对他人造成伤害或对他人及公共财产造成破坏、损毁，乙方应自行承担因此产生的一切责任。

乙方不允许带贵重物品、大量现金参营，违反本协议强行带入夏令营者，发生丢失或损毁事故，甲方不负任何责任;乙方应注意妥善保管自己的证件及行李物品，如发生随身携带行李物品遗失、被盗等情况，应在第一时间报告甲方辅导员老师，并保护好现场，等待有关人员处理。如果有涉及保险相关的问题，乙方应保留所有相应的单据和证明文件，以便进行保险理赔工作。

甲方拍摄的参加训练营员在训练中的相片、DV等资料，甲方可无偿用于宣传、招生等用途。

夏令营期间，原则上不允许乙方提前离营，若因特殊原因需提前离营者，须由乙方监护人提交书面申请，签字确认后将其传真到z，待甲方确认批准后方可独自离营，乙方若在独自离营后遇到任何安全问题和意外均与甲方无关，甲方对此不承担任何责任。

五、其他事宜：

1、退费说明：乙方报名缴费后，凡因乙方自身原因不能参加夏令营，其夏令营报名费按以下原则进行申请退还。

①退费原则

开营前_______天(_______年_______月_______日)之前(不含当天)申请，退还_______%报名费;

开营前_______天(_______年_______月_______日)之前(不含当天)申请，退还_______%报名费;

开营前_______天(_______年_______月_______日)之前(不含当天)申请，不予退费。

②申请方式

需以书面材料进行申请。(退赛申请书+营员身份证复印件+监护人身份证复印件)传真至组委会(z)方有效。

③申请时间

以组委会接收到申请材料时间作为退营申请时间。

2、请假说明：乙方参营后因故请假的，甲方不做其他补充培训和退费补偿。

3、本协议未尽事宜，双方通过协商解决。若协商无果，则按国家有关法律法规办理。

4、双方因本协议发生争议的，由双方协商解决。若协商不成，则任何一方均有权向甲方所在地有管辖权的人民法院提起诉讼。

5、本协议一式两份，甲、乙双方各执一份，具同等法律效力。本协议自双方签字(盖章),在乙方交付本协议约定的全部团费后自动生效，至双方权利义务均履行完毕止。
